Care and support is the term used to describe the help you may need at any age to live as well as possible. It can mean anything from help with things like washing, dressing and eating to getting out and about, getting back into education or keeping in touch with friends or family. You may need care and support if you have a mental health or long term health condition, learning disabilities or physical disabilities, a dementia type illness, issues with drugs or alcohol or a sensory impairment. You may have one need or complex care needs.

You may receive care and support in your own home, or you may live in a care home. There are lots of ways we can support you to live an independent life including all the activities, groups and services available in our local area.

We want to be sure that you feel in control of any care and support you receive now and in the future. When deciding what help you need, make sure you think about what is most important to you. We want to support you to remain independent and healthy for as long as possible.
