Children's Centre Services Request for Support

This form is to request support for a family in Bath and North East Somerset with a child up to 5 years of age (or 10 years of age if a Parenting Programme, or Theraplay is being requested). Places are limited by capacity and available funding).

Children's Centre Services offer family and home learning support, employment skills and a variety of groups for families with children aged pre-birth to 5 years.

For some services (e.g.: Theraplay, Family Links parenting programme, Freedom) the service may work with families with children aged up to 10 years of age, where capacity and funding allows.

By filling in this form you’ll be helping us decide what type of support is most appropriate for the children and family.

Before you start

Make sure the child(ren) live in Bath and North East Somerset. Check to see who their local council is.

To complete your request make sure you have:

  • Signed consent form from the family
  • Further guidance on sharing protocols
  • The name, date of birth (or estimated date of delivery) and address for each child you are including in the request
  • The name and contact details for each parent or carer
  • Information about what support has been offered to the family
  • Electronic copies of previous assessments to upload with this request
