SEND Travel Support

Travel Support means any eligible support to ensure that children/young people are able to attend school.

Use this form to contact the council about SEND Travel Support including new applications and changes in circumstances.

Please provide as much detail as possible and have all the information to hand, including any supporting documents saved to the device you are using. The information you provide will be used to decide which travel support option is available to you.

Information you will need includes:

Details of the child/young person
Child/young persons' home address and name of educational setting
Your email address and/or phone number
Electronic copies of any documents to upload with this request

Please ensure all parts of this form are completed in full as missing information may delay processing your application. Give yourself enough time to complete the form as you cannot save your progress.

You must apply by 30th June if you want travel support to start from the beginning of the next academic year
