Use this page to find out about the support you can get between the ages of 21 to 25 as a care-experienced young person in Bath and North East Somerset.
From age 21 to 25 the offer can change slightly. We will talk to you about what happens before you turn 21. We will ensure that nothing will change without your knowledge.

If you still require support
- If you continue to need support, in any area of your life, we can continue to support you and review and update your Pathway Plan together.
- A lot of the content in this offer is available to you post-21, which we can discuss with you at any point.
- We will be in touch with you at least once a year if we haven’t heard from you.
If you no longer require support
- If you are managing well and/or are not meeting your Personal Advisor (PA) regularly we may decide to ‘step down’ support so that we are there in the background for those times you may need us – you could call your PA or the duty number to let us know what support you may need, when you need it.
- You can ‘opt out’ of support post-21 and tell us you do not want any support from the Care-Experienced Team. If you change your mind and want to ‘opt in’ then you are welcome back at any time.
