The purpose of processing

We will use the information provided to help us with the following:

Whether you require an assessment under the Mental Health Act

To help inform any assessments or reviews where you may already be subject to the Mental Health Act

If you are being assessed under the Mental Health Act we are required to consult who is considered to be your Nearest Relative under the Mental Health Act (different to next of kin) e.g. this may be a parent, sibling (over age 1), grandparent etc.

Following assessment and/or review we may need to make referrals to other agencies such as other mental health teams, social care, Safeguarding etc.

Data Subjects

Children and adults referred to the AMHP Service for an assessment under the Mental Health Act

Personal Data

The personal information we may collect in order to decide if a Mental Health Act Assessment is required may include the following:

Your name, gender, NHS number, Date of Birth, ethnicity and disability etc.

Any information we may need to take account of such as communication needs, religion, and past medical and mental health history

Information shared by any relevant professionals involved in your life such as G.P., Care Coordinator, Social Worker etc.

Colleagues from other organisations we may need to work with in order to assess your needs e.g. your care coordinator, Consultant Psychiatrist, G.P. and other healthcare professionals.

How long is the personal data retained by the Council?

Personal data will only be retained for as long as it is needed for the purpose specified above, or as required by applicable law or regulatory requirements.
