The purpose of processing
The purpose of collecting personal information through the LDIS model is to validate reports and to make contact with yourself should additional information be required about the report that has been made. We will process your data in order to gather intelligence about emerging, dangerous practices or substances in Bath and North East Somerset, so to enhance the local response and inform professionals who work with individuals who use drugs, of the immediate risk.
Data subjects
Data subjects are individuals who fill out the LDIS form.
Personal data
We will collect the following information: Reportee name; organisation the reportee works for; job title of the reportee; email address of the reportee; telephone number of the reportee.
How is it used?
We will use your telephone number and/or email address to contact you regarding the report you have made, and to validate the report and ask for further information if required. We may also use the email address you provide to share meeting invites with you regarding the report.

Sharing of personal data with external recipients
The reportee organisation and job role will be shared with individuals on the LDIS panel, so that they are aware of where the alert has been raised from and who the services may cater to.
How long is the personal data retained by the Council?
We will retain your personal data for no longer than five years.
