How is it used?
We will record your information using the webform or where necessary by phone. That information will be handled by the Assessment Team who will verify your identity and carry out a check using databases provided by the NHS and the Department for Work and Pensions to ensure that you are eligible for the payment. Once we have confirmed the above we will transfer the payment to you.
