If you want to open a zoo, you must complete this form at least two months after you have given us notice of your intention to apply for a licence.

Completing this notice form should take no more than 10 to 15 minutes of your time.

You must pay the application or renewal fee before we can process your application.

You must also provide:

• your notice of intention reference number
• details of any significant changes to the information you provided in your notice of intention

Supporting documents

As part of the application, you'll need to provide:

• a copy of the local newspaper which published your notice of intention
• a copy of the national newspaper which published your notice of intention
