Health and wellbeing

Use this page to learn about the support that we offer care-experienced young people regarding their health and wellbeing after leaving care. 

Your health is a priority, both physical and mental, and something we will always consider with you. We will ensure that you have opportunities to access services that meet your needs and support you to manage your health and attend your appointments if you need this.

Immunisations and health passport

You will be provided with your health history upon leaving care; this will include your immunisations, so you know what you have had. 

GPs, dentists and opticians

You should be registered with a GP local to you. Your GP is a gateway to most other health services, and your GP can refer to services such as talking therapy, mental health services and other specialist health teams you may need.

You should already be registered with both a dentist and optician, but if you are not, we will support you to register.

Leisure

We will support you to access hobbies and interests that you have within your local community. 
You can get 50% off Pure Gym Memberships. You can also get 33% off Better Gyms in B&NES.

Ask Jan membership

You will be offered an ‘Ask Jan’ membership, which gives you access to online and telephone support (including counselling), advice, guidance, and webinars, as well as access to discounts via a MAX card. You will need to sign up for a membership. 

Mental health services

We can advise you about primary and secondary mental health services. Your GP would usually be the first port of call for accessing mental health services. Some services that you could access include Vitaminds, Samaritans, Bath Mind, and Wellbeing house in Bath.

Sexual health

We will support you to think about contraception, sexual health, and healthy relationships.

Drug and alcohol services

Project 28 and DHI are services in B&NES that can offer you support. Other local services will be available if you live outside of the area, which we will support you to access.

Adult care

If you have adult care and support needs, you are likely to already have an Adult Social Worker. We will ensure we work with them and alongside you.

Feeling safe

If we are worried about you, we will act on this with a view to keeping you safe - we could involve the police, health services and adult safeguarding.
