The Annual Reviewing team provide a service where the care funded by Bath & North East Somerset Council is reviewed annually to make sure support needs are being met.
If a service user receives a package of care that has been funded by Bath & North East Somerset local authority (both in and out of county), the Annual Reviewing Team undertake scheduled annual reviews of the care that each individual receives.
What the team do
When conducting an annual review, the team look at an individual's care and support needs and how they are being met by the use of commissioned services and community assets.
The team also complete assessments and reviews of carers for service users living in Bath and North East Somerset.
The Annual Reviewing team are responsible for:
- reviewing people with a package of care that is funded by Bath & North East Somerset Council
- completing mental capacity assessments in relation to the provision of adult social care
- amending support plans as required to include the addition of services to meet eligible need, or the reduction of services when needs have changed or reduced
- undertaking assessments of need for informal carers when it is flagged as necessary during the annual review of the person receiving care
- occasionally undertaking Care and Support Needs Assessments when it is flagged as necessary during an annual review
Social Care Practitioners will make referrals to the Safeguarding Adults Team following any safeguarding and self-neglect concerns (following the B&NES Community Safety and Safeguarding partnership (BCSSP) Multi-Agency Safeguarding Adults Procedures and BCSSP Self Neglect Policy and best practice guidance)
