Transitioning from Children's Social Care to Adult's Social Care

As part of Preparing for Adulthood, some young people may require an assessment from Adult Social Care, this is called a Care Act assessment.  

If you're struggling to do things daily and think you might need extra support from social care, a Care Act Assessment is the first step.

Care Act Assessment

A Care Act Assessment is free, and you're entitled to one regardless of your financial situation or level of need. The Care Act Assessment works out what kind of extra care or support would meet your needs and can include:

  • adaptions to your home
  • practical help from a paid carer
  • moving into a care home

You might be asked to contribute to the cost of some of the services recommended by the assessment, but this will depend on your situation and the type of service recommended.

Who can get a Care Act Assessment

Anyone who needs extra support might need a Care Act Assessment.

This could be if you, or someone you know, has:

  • a learning disability
  • a physical disability
  • mental health needs
  • drug or alcohol issues

You could also get a Care Act Assessment if you, or someone you know, has difficulty with the following activities:

  • managing and maintaining nutrition
  • managing personal hygiene 
  • managing toilet needs
  • being appropriately clothed
  • making use of your home 
  • maintaining a habitable home environment
  • developing and maintain relationships
  • accessing and engaging in work, training, education or volunteering
  • making use of necessary facilities or services in the local community, including public transport, and recreational facilities or services
  • carrying out any caring responsibilities
