Learn about the Adult Social Care Service

Our Adult Social Care Service help to assess and support adults in our community, helping people to live happy, independent lives for as long as possible. Use this page to learn about the service we offer and how to access it.

What we do

We assess and support the needs of older people, disabled adults of working age or those with complex needs. Our aim is to promote independence to sure we are protecting vulnerable people and enabling individuals to remain in their homes, reducing the need for long-term care or hospital admission. 

Our team of Social Workers and Care Service Co-ordinators will discuss your needs with you and offer you information and advice.

If appropriate, we will offer you a Care and Support Needs Assessment and help you to plan the support you need. This applies to people who are based in Bath and North East Somerset and who live in their own homes, or in residential or nursing care. We work in conjunction with other local teams and wider community services. We also offer support and assessments for carers.

How we support you

Our aim is to promote your wellbeing and to help you live independently for as long as possible. We will help you to access the support you need, this may include:

  • offering you a needs assessment
  • helping you to plan your support
  • referring you to our Reablement Service to help you maintain your independence
  • referring you to a domiciliary care service as part of your care and support plan
  • signposting you to local support groups
  • helping you set up a direct payment to purchase your own care and support

How to access the service

You can refer yourself to the Adult Social Care Service, or you can be referred by your GP or other health care professional.
