Use this page to express an interest in delivering services in your General Practice.

We wish to continue the Public Health services delivered by General Practices, which include the following:

  • NHS health checks
  • Smoking cessation support
  • Long Acting Reversible Contraception (LARC)

HCRG Care Group will cease subcontracting these services on behalf of our Public Health team on Monday 31 March 2025. We want to have new, direct contracts between our Public Health team and General Practices to be in place from Tuesday 1 April 2025. We intend to award these contracts for a minimum of four years to 31 March 2029, with the potential for a further extension of two years to 31 March 2031.

Services within the contract

NHS Health Checks

The NHS Health Check is for people who are aged 40 to 74, with some exceptions. The check helps to assess the risk of getting certain health problems, such as heart disease and stroke. The check measures:

  • height and weight
  • blood pressure
  • cholesterol
  • physical activity levels
  • alcohol consumption
  • smoking status
  • family history of coronary heart disease

Smoking Cessation

The smoking cessation service offers behavioural support and advice to people who want to stop smoking. It also includes access to stop smoking aids that increase the chances of quitting for good. This includes:

  • nicotine replacement therapy
  • medications
  • e-cigarettes (vapes)

Long Acting Reversible Contraception (LARC)

This service offers the fit, monitoring, checking and removal of Intrauterine Contraceptive Devices (IUDs/IUSs) and Subdermal Implants (SDIs), providing women with a highly reliable form of contraception and means of preventing unintended pregnancy. The fitting of a LARC an provide effective contraceptive protection for up to five years depending on the device fitted and improves women’s choice and control over their reproductive health.

How to express your interest in delivering services

Follow the steps in the guide below to apply for a new contract to deliver these services.

  1. Step 1

    Download and read the detailed service specifications for each service you are interested in providing, to familiarise yourself with the individual service requirements.

  2. Step 2 2

    1. Download the expression of interest form

      You should complete all sections in full. You can do this electronically and save the document, or print and complete the form by hand, then make a scan of your completed form.

      Email your document or scan to public_health@bathnes.gov.ukby 5pm on Monday 10 March 2025.

  3. Step 3

    We will review your completed form and respond to you directly if we have any queries on your application.

  4. Step 4

    If there are no queries, we will confirm your eligibility to provide the services.

  5. Step5

    We will provide a contract for the services and send this to your general practice.

  6. Step 6

    You should review your service provision contract, sign and date it. Email the signed and dated document back to us at public_health@bathnes.gov.uk

  7. Step 7

    1. Once we have received your signed contract, we will confirm how your practice should submit quarterly services data and how you will receive payment.

  8. Step 8

    We would like you to provide the services from Tuesday 1 April 2025.
